Appointments
Appointments can be made simply by calling 503. 901. 2075 or by email on the contact us form. Your initial intake forms will be emailed directly to you to fill out prior to your appointment. Please have these returned by email at least 24 hours before your appointment.
Appointment Policy
Your appointment time is a reservation of the room for you and I make every attempt possible to run on schedule in respect of your time. Occasionally, there is a problem when patients aren’t used to staying on a schedule. Please keep in mind if you are going to be more than 15 minutes late for an appointment to call to confirm availability. There is a 24 hour cancellation policy on all appointments which allows time to schedule another patient that would benefit from treatment. This appointment policy allows us to develop a mutual consideration and respect for my time and yours.
Payment Policy
Payments can be made by check or cash.
A “superbill” can be provided for you to submit to your insurance company. This allows me to devote more time and resources to you as a patient.
